Information Form & Next Steps

Complete this form once to get started as a group participant. I’ll review your information and follow up with next steps. Before your first session, you’ll need to review and sign a participation agreement outlining confidentiality, client privacy, group expectations, fees, attendance policies, and the scope of consultation.

Please be prepared to provide your EMDR Basic Training completion certificate, if applicable. If you are still completing Basic Training, please provide your training provider and expected completion date.

Once your information form and participation agreement are complete, you can sign up for individual sessions as they become available.
